ABILITYWorks Application Checklist
Before beginning your application, please have the following information available:
- Service provider, case manager, job coach, or follow-along support email address (if applicable)
- Employment history and employer contact information (if applicable). Previous or current employment is not required for admission to the ABILITYWorks program.
- Two (2) professional reference email addresses. References must have directly observed your performance and be able to evaluate all areas of the reference form based on their experience with you. References may not be relatives.
Digital Documentation Needed for ABILITYWorks Application
Eligibility Documentation
Upload one of the following:
- Most Recent Psychological Evaluation
- Most Recent Psychoeducational Evaluation
- Most Recent Medical Evaluation
- Equivalent documentation from a qualified professional or agency
Documentation must support the criteria for an intellectual and developmental disability (IDD).
Educational or Functional Support Documentation
Upload one of the following:
- Individualized Education Program (IEP), including a Behavior Intervention Plan (BIP), if applicable
- Service Provider Verification documenting services received, functional abilities, and support needs
- Employer or Supervisor Evaluation documenting job performance and level of independence
Documentation must be sufficient to demonstrate program eligibility. Additional documentation or information may be requested during the application review process.
